PATIENT INFORMATION – TAKING CARE OF YOU
We try our best to make your visit both comfortable and pleasant. In an effort to better acquaint you with our office policies, we have added some patient information for you to look through such as forms, financial information, FAQ’s, and information regarding colonoscopy. If we have not answered all of your questions here or you have any additional questions, please do not hesitate to contact our office at 303-840-8822.
YOUR APPOINTMENT
Our patient forms are available to download and fill out prior to your first appointment. If you are not able to fill out the forms beforehand, please plan on arriving 10-15 minutes early for your appointment. If you bring your forms with you, we will only need you to arrive about 5 minutes early. Please bring along your driver’s license, insurance ID card and a list of current medications to your appointment.
Referrals from your primary care physician are only required if your insurance company requires them. Check your insurance card, your policy handbook or call your insurance directly for clarification.
If you feel that there are any records that would be helpful for the doctor to see the day of your appointment, please bring them with you or have them faxed to our office at 303-840-8824.
